NEMT Driver Training in Illinois: The State-Approved Safety Training Requirement Explained
Illinois NEMT driver training explained: the state-approved safety training certification medicar and service car providers must have, the three-year cycle, and where online courses honestly fit.
The requirement: a Department-approved safety program every three years
Illinois handles NEMT driver training differently from almost every state we have covered. For Medicaid medicar and service car work, the required safety training certification must come from a training program approved by Illinois Healthcare and Family Services (HFS), not from any vendor a company happens to choose.
Under Public Act 95-501 and 89 Ill. Adm. Code 140.490(f), providers of non-emergency medicar and service car transportation must certify that all drivers and employee attendants complete safety training certification every three years.
- Safety training programs must be approved by HFS, which maintains the current approved-provider list.
- Training must cover passenger assistance, vehicle operation and passenger safety, and emergency procedures for drivers and employee attendants.
- Certification generally cannot be issued by an entity affiliated with the transportation provider, with an exception for IDPH-licensed providers that obtain HFS approval for a qualifying in-house program.
- Providers must keep completion documentation available to HFS on request. Providers with qualifying federal funding under 49 U.S.C. 5307 or 5311 are exempt during the funding period when they maintain the required documentation.

What Illinois companies may look for beyond the state requirement
The HFS safety certification is the regulatory centerpiece, but Illinois NEMT employers and their contracts may add background and driving-record checks, drug screening, and other training documentation. Exact requirements vary by company, contract, and program; ask for the full list in writing.
Unlike some direct-enrollment states, Illinois's safety-training rule does not itself name HIPAA training. Privacy training may still appear in a contract, so confirm rather than assume in either direction.

Can I take my Illinois NEMT safety training online?
Only if the program is on the HFS-approved list; check that list for each provider and its delivery format. Cascade Horizon Academy is not on the list, so our online courses are supplementary knowledge training, not the required certification.
How often does the safety certification renew?

Who approves training programs in Illinois?

Can an IDPH-licensed transportation company train in-house?
The rule allows an IDPH-licensed medicar or service car provider to seek HFS approval for an in-house safety program that meets the required components. Approval comes from HFS.
